At Jackson Eye Care, we believe in providing you with personalized, professional and convenient care as well as access to the latest technology and treatments. We accept new patients every day, and urgent or emergency referrals are seen the same day.

What to Bring
Please bring the following items to your appointment:

Photo ID & Insurance Card(s)

List of all current medications, including vitamins and supplements

Your health history — conditions, procedures/surgeries, allergies, etc.

List of any questions or concerns about your eyes or vision
In addition, we always suggest that you bring someone to drive you home since your eyes will remain dilated for several hours. Sunglasses may help reduce glare. If you need some, please ask at the front desk before you leave. If needed, our receptionist can also help you arrange for transportation.
New Patient Registration Packet
As a new patient, you will be asked to complete a New Patient Registration Packet. As a way of making your patient experience more efficient, please print and fill out the forms in the convenience of your own home. You can access the forms by clicking the blue button below.
Please remember to bring the completed New Patient Registration Packet with you, when you come in for your appointment.

Your Visit
What to Expect

Treatment
If treatment of your eye problem is indicated, you and Dr. Jackson will decide the most appropriate course of action. Some types of treatment can be performed in our office at the time of your visit. Others may have to be scheduled at a local outpatient surgery center or hospital.
Follow-up Visits
If you are coming to our office for a follow-up appointment, your visit can last up to two hours, depending on the specific testing and treatment you need. For your safety, we recommend that you have a friend or family member drive you home from any appointment involving treatment or procedure.
Fees and Insurance
We are participating providers for many insurance plans, including Medicare. Prior to your appointment, please check with your insurance provider or contact us at (862) 904-8349.
